How updating a mod game works?

This one really confuses me (I'm a newbie) so let's say I have an old modded game and then suddenly there was an update and the new update was modded as well. What would happen?

A) It will update automatically and my saved game will not be touched(erased).

B) I have to update it manually and my saved game will not be touched(erased).

C) It update automatically and my saved game will be touched(erased).

D) I have to update it manually and my saved game will be touched(erased).

E) Other...
